Outreach Case Management Program

Aged-out foster youth are among the most under-served, disconnected and statistically vulnerable segment of our society. These young people often lack the skills and the self-esteem needed to become self-sufficient adults. Grant funds expanded the Outreach Case Management Program, aiming to double the number of youth among their non-residential clients and increase the variety of services offered to them.
